Tesamorelin is a GHRH(1-44) analogue with an N-terminal trans-3-hexenoic acid group that resists DPP-4 degradation and extends stability. In vitro it binds the GHRH receptor on pituitary somatotrophs and is studied as a secretagogue that drives endogenous, pulsatile GH release rather than supplying growth hormone directly.
